Don't loose hope you have come this far, if you are not getting interviews, you should look over your application and see where you are lacking. You can then look toward what you need to do this following academic year to move ahead. If you don't have a green card there is not much that can be done, and being depressed over this helps no one, instead you can study hard and pass step 3 to make up for this point. If you have been away from clinical medicine for a few years you can obtain USCE and get strong letters of recommendation to show that your skills are current.
Even ones accent can be a barrier everyone has a unique profile, so sit back review your application and take this year as an oppurtunity to strengthen your application. Many of us have been in the same boat.
