International medical graduates may attempt to enter fellowships after having completed an ACGME accredited residency in a primary specialty, or may attempt to enter a fellowship based on specialty training outside of the United States.
Not all fellowships are accredited by the ACGME. Those that are accredited are listed in the Graduate Medical Education Directory ("the Green Book") and in the On-line FREIDA on the AMA Website where the same type of information provided for residency programs can be found. Some, but not all, accredited fellowship programs participate in special Matches conducted by the NRMP. These include the Combined Musculoskeletal Matching Program and the Medical Specialties Matching Program, as well as a dozen or more matches for individual subspecialties. These programs operate very similarly to the Main Match for residency positions although the deadline dates vary. International medical graduates may enter into fellowship programs that are not accredited by ACGME, but depending on their citizenship and immigration status, may need to make special application to ECFMG (see Visa Issues for Fellows) |

It is possible for IMG to get a non-Accredited fellowship in some states. But you have to have a green card to be able to issue a limited lisence in that state (max for 2 years). After all, completing your training you will not be able to get a Board Certification thus this training is not recognized, so you will not be able to work in the US.
